In these classes you will learn how to live a healthier life with diabetes:
What to expect:
A trained instructor leads the class, which meets several times. Your class will be small - probably no more than 20 people. You will have plenty of time to ask questions and learn more from others in your class.
Classes meet once a week for six weeks. These classes are intended for, but not limited to, Medicare beneficiaries who have diabetes or are pre-diabetic. Below is a list of upcoming events in your area and how to register. Click on the column headers to sort.

Have you seen the effects of chronic illness up close? Do you want to be part of a nationwide effort to help people with diabetes feel better every day? You are invited to participate in the upcoming Diabetes Empowerment Education Program (DEEP) Peer Educator Training. This opportunity is offered by TMF Quality Innovation Network and Primaris, as part of the Health for Life, Everyone with Diabetes Counts initiative.
This project is part of a national health disparities program, funded by the Centers for Medicare & Medicaid Services, to address the higher numbers of hospitalizations, complications and death related to diabetes found in certain communities. We are pleased you will join us in the fight against diabetes. Together, we can save lives.
Primaris and CLAIM will provide the training and provide the tools and resources for no charge to help community partners deliver these classes in settings where neighbors, relatives and friends can benefit. Once trained, partners will run a six-week series of classes with activities and exercises that help people understand how to make better lifestyle choices.
